Dr. Michelle Cameron-Donaldson and Regenerative Orthopedics Utah are required by law to:

By consenting to a video visit, you acknowledge that the media, link, or software used may not be fully HIPAA compliant due to emergency provisions. Regenerative Orthopedics Utah will take all appropriate and available actions to help keep these visits safe and secure.
Informed Consent
By consenting to care, you acknowledge that it is your responsibility as a patient to seek out information needed regarding your diagnosis, management, treatment plan, and medications. You agree to be a partner in your health care and in decisions related to your care.
Complaints & Information
You may submit written complaints regarding potential privacy breaches or requests for further information by contacting us directly.
Regenerative Orthopedics Utah reserves the right to change its information practices and to make new provisions effective for all PHI we maintain. Revised notices will be made available on our website.

Program Description
Regenerative Orthopedics Utah may send SMS text messages to patients and prospective patients who have provided their mobile phone number and opted in to receive communications. Messages may include appointment reminders, scheduling confirmations, health information, and practice updates.

By providing your mobile phone number and agreeing to receive text messages from Regenerative Orthopedics Utah, you expressly consent to receive SMS communications from us. Consent is not a condition of receiving medical care or any other service. Message frequency may vary.

Message and data rates may apply depending on your mobile carrier and plan. Regenerative Orthopedics Utah is not responsible for any charges incurred from your mobile carrier for receiving SMS messages.
How to Opt Out
You may opt out of receiving SMS text messages from Regenerative Orthopedics Utah at any time by replying STOP to any text message you receive from us. After opting out, you will receive a single confirmation message and no further SMS messages will be sent.
